I got Dysport for my jaws and forehead with Heather. I was apprehensive as it was my first time, but heather is so knowledgeable and really helped me to be at ease for the procedures. She was wonderful. My face looked younger with in days as my forehead wrinkle disappeared, but I still have good expression. Will definitely come back again!

I came to Dr Chidester for liposuction and I am so happy I chose to come here! His bedside manner is so amazing. The staffs' hospitality and care makes you feel so at ease. After my surgery my gal pal got sick and when I came in for my 24 post op, he gave us both IV's and prescribed us a zpac out of the kindness of his heart! He always answers so quickly and answers all questions I have no matter how silly or weird. I had an area I was still concerned with after surgery and he is doing a touch up for free!!! I would suggest Jerry Chidester and Form Spa looking to get plastic surgery or even just as a medical spa!

I have contemplated getting my deviated septum fixed for the better part of the past 5 years. I had several deviations resulting from a car accident and a surfing accident. I met with several doctors for consultations in several states trying to find the one I felt the most confident in. Some refused to do the surgery due to the severity of my case. Others felt competent, but were aggressive in pushing me to get it done. As a doctor myself I felt a heightened sense of watching their bedside manners and listening to their explanations of the procedure. I place high value on patient autonomy, a patient’s right to know what is going on with their health. Dr. Ward was just a step above his colleagues in the field, and was very respectful of this being my decision. He took the time to express to me the details of my case; taking care to make it understandable. I loved that they were able to show me at the first visit what they anticipated my nose would look like after the procedure. The office did not hassle me, but rather was there to assist with questions leading up to the procedure. Dr. Ward took time to call me on his cell phone after hours to help answer some last minute questions. He was transparent and available to me. I really appreciated that. I felt very confident I had found the best surgeon I could to fix my nose. The team that worked with me the day of surgery was fantastic. They all were there for one reason, to make me comfortable. There was top-notch communication regarding my post op instructions for my recovery. I don’t know why septoplasty gets such a bad rap regarding the recovery. Was it a joy? No. But it was very manageable. I credit Dr. Ward and his team for that smooth recovery. Post-op visits have been great. I love seeing the changes to my appearance. It just keeps getting; closer and closer to the picture they showed me at that very first visit. I am amazed at how much better I can breath. My only regret is that I did not find Dr. Ward sooner. Lucky for you I did all the legwork for you! I cannot recommend Dr. Ward and his team highly enough. He is passionate about his career, caring, kind, and a superb doctor.

After a great consult with Kylee, she addressed me concerns and we ended up scheduling an IPL as my main concern was pigmentation. The very worst part, which wasn’t terrible, was about 30 minutes afterwards, which Kylee warned me about. It just felt like a bad sunburn. I ended up having a lot of pigment come up to the surface, and after about 1 week, it all stuffed off and my skin tone is so much more even. I love it. I can’t wait to do it again to get it even more smooth! Kylee was so great through the whole process. She explained everything so well and did a great job! I love Form Spa!!

This is my second time getting a hydrafacial with Kat from FormSpa in Cottonwood Heights. I’m SO glad that I was stubborn enough to find a slot to schedule with her since I heard so many good things about her and liked her personality already from seeing Instagram posts. Kat is not only super accommodating, open minded, kind, understanding and sensitive to one’s needs, but also very knowledgeable when it comes to skin. Me as a newbie in this area loves knowing more about it and what’s best for my skin personally. She never judges a client and I feel like she’s a very honest and trustworthy person. I’ll keep going there and will stick with Kat since she knows me now with my specific needs. If you haven’t gotten a hydrafacial, I highly recommend it, everyone who’s trained to do them at formspa is greatly qualified for it!!

Make sure you know they do NOT give refunds-even when they are in error. I honestly had the best experience with my first visits, but it all fell apart in the last month. I went for a laser hair removal treatment on my legs. The tech could not get the laser and cooling air to work properly. I expressed that my legs were burning and she told me it was normal. I told the front desk before leaving that they were on "fire" and told to see how they felt when I got home. I called later that day crying telling them I was burned. Again I was told that it was normal and to take benadryl and put on hydrocortisone. I have gone from burns to looking like mud was spattered on me to now having discolored circles. The doctor said it will take weeks to recover (I believe months). I asked for a refund for my remaining services. I was told that they offer NO REFUNDS. I was lucky to get a "spa credit" for unused services. I did nothing wrong, yet I have been treated like what happened was something I did.

I've been into Form several times and have always been overly impressed by how professional, intelligent, and well informed the staff is - and just an overall good-feeling the entire place gives off. I'll admit, I was very nervous (and pretty self conscious) as a guy to get Botox and filler - now I only wish I had done both sooner! The absolute best part about Form is the no pressure environment. I've never felt that I needed to do anything or buy anything unless I wanted to. If you're considering any of their services, stop it for a consult and ask for Shelby - she'll make you feel right at home.

I want to give better but my experience was horrible. This place doesn't even know titles or what's going on from the inside to even deliver proper customer service. I spoke to a woman named Autumn who claimed to be the owner who was very rude and told me numerous times I would be called back and never did, not once. Than when I called in another time I spoke to Jordan I think it was who was wonderful to speak with and knew a lot about everything that was offered, but was told Autumn is not the owner and that another lady (Channa I think) is the GM over it all. Seriously! What company doesn't have structure or know their titles? Or lies about them! They definitely lost a prospective client and I'm only giving two stars for the kindness and knowledge I was given by Jordan, make her GM. At least she answered the phone with a nice tone and was a joy to talk with.

First off I want to start off will all of the good things about the office, because even with the short comings, I have still continued to return. The office is gorgeous, clean and everyone is always really friendly. But there needs to be some sort of organization overall. I have had surgery here twice and one time I was already back and had an IV when they realized they were missing some paperwork and my payment info, even though I had already paid and filled everything out. It had gotten lost and I had to fill it out. And almost every time I go they don't have my paperwork from before and I have to re-fill it out. And most times I call about something, I get told I will get a call back from someone and never do. I feel like there is a big disconnect between their surgery department and their front desk. Doing business somewhere should be easy, but they unfortunately make it really difficult and I have to constantly be on top of it. Although I am sure it is unintentional, it is still a pain.

I came here for much needed rest and relaxation. A 90-min massage turned out to be just that. As soon as I walked in, I knew I'd have a comfortable experience. The spa itself is clean, bright & organized, and the staff were pleasant and accommodating. Once in the masssge room, Sarah asked me exactly what areas seemed tightest and how much pressure I preferred. Following these guidelines, she helped work out some long existing knots. I'd definitely come back again! Plus, the list of services provided here is extensive!

I've been going to Form Spa for almost a year now, but this was my first experience with Rickale at the Riverton location. I had a hydrafacial, and I appreciated that she took the time to get to know my skin care concerns, and she was so thorough in my treatment. I also loved nerding out with skin care talk. I'm only docking a point, because I didn't love the open concept of the treatment. I'm used to the Cottonwood location with much more privacy, and that fits my personality better as well. Rickale told me next time she'd just set me up in the private room. Last thing, I'd like to try out some of the form products, but I wish the website had more info. I love to research products, and the ingredients and their potencies aren't listed on the site. For example, the bright eyes cream says there are 5 active ingredients, but they don't list what they are! For $110 for .5oz, I'd love to know exactly what I'm buying, maybe read some reviews? I'm a skin care junkie, and I've only been holding off, because of this. Other than that Form Spa is a dream, and I love going! I'm doing Microneedling with PRP next! -Lindsey
