Not a positive experience for my father. His Alzheimer's condition brought him to the ER displaying some combative, aggressive behaviors. He was referred here by the hospital doctors as an interim facility pending long term plans for care. This was our family's first experience in this area. The facility made a lot of promises going in, few were backed up. We were promised medical updates from doctors. That never happened. After admitted, he was in someone else's clothes though we provided clothes for him. We brought it up several times and we're assured they would check on it. That never happened either. He also had some sort of seizure while there which was a first. This did not seem to be documented in any way which I find unbelieveable ! We finally pulled him out early. We were given a basic release packet with little info. This place is just a human warehouse.

Every time we were there for my father-in-law, the staff was very nice and the facility seemed clean. However, they lost one of his hearing aids. They have refused again and again to call me back or admit their fault in losing a medical prosthesis. I've even attempted to contact their corporate headquarters in South Bend and THEY also failed to call me back. I could understand if socks or shirts go missing, but not a prosthetic. I'm ready to file suit against them to pay for the replacement. There has been no follow-up and no accountability whatsoever. I'm beyond frustrated.

I was referred to this facility for physical and mental therapy. The physical therapy consisted of a 15 minute session on how to use a walker, but no walker was provided after that. The mental therapy consisted of the entrance interview with the chief psychiatrist and then daily brief sessions with an assistant. I confess that I lied during the latter sessions because I was so eager to get out of this place. There were also group sessions, but many of the patients were unable to participate, and the psychologist leading them offered very little concrete advice. The food was ok, but often cold and of very small portions. It was difficult to get showers or even to get a toothbrush. It was difficult to get the staff to deal with problems. For example, one of my roommates clogged up the toilet in our room. It was nearly a week before it was cleared. During that time we had to use another room's bathroom. There was only one television in the facility, and the choice of what was on was nearly always that of the staff. There were few activities for patients. I am a great reader, and it took several days and much pleading for me to get access to books friends brought me. The worst thing was that I was very afraid during my stay. Some of the patients wandered freely in and out of rooms. Others shouted and screamed in the hallways. A few members of the staff were compassionate and competent, but most were overwhelmed, and several were scary. The facility was run in a way that was for the convenience of the staff and not the patients. At one point patients were awakened at 4 AM on a Sunday to be weighed. It took a very long time for me to be discharged. At one point the facility wanted to transfer me to another located 3 hours away. When I was finally released, the facility couldn't find several items of my clothing. I was so eager to leave I didn't press the issue. The upper levels of the staff had little interest in me until just before my departure. Then, the psychologist criticized the arrangements friends and I had made, but she offered no advice or assistance. This was not a good place. Avoid it.

I was put here for about 48 hours and that was all I could stomach. The usual minimum is 5 days, but fortunately I got help getting out. I was stuck in the dementia unit- I am an 18 year old girl with depressive issues, not alzheimers. The windows are highly secured. I did not get fresh air the whole time. A girl I met was also stuck in the same ward as me that shouldnt have been there- only she was there for over two weeks, eating always-cold food, around old women, never getting fresh air. The nursing staff is nice enough if you're in your right mind, but they're apathetic to the older women who don't know whats going on. This place is not a place of healing.

PLEASE DO NOT send your loved one here, especially if they have other medical issues!!!!! They may have a good psychiatrist, but the staff did not take care of my mom's feet and legs. They were suppose to be wrapped because of severe edema. I noticed the first time I visited her they weren't wrapped. When I asked why I was first told that with it being a psychiatric hospital, they usually wait 24 to 48 hours to make sure she's not going to harm herself or anyone else with the wraps. When I went to visit her again a few days later I noticed they still weren't wrapped. This time when I asked about it I was told they didn't have orders for it, which was a lie because the nursing home she came from sent the orders and the wraps. I called the nursing home to have them send the orders again, which they did. I was then told that the doctor ordered compression hose instead. A week later when I visited, she still had nothing on her legs. I complained again and was told they were still on order, and that they were going to send someone to CVS to get some. She did have compression hose on when I visited the next day, but now the staff was failing to take them off at night like they're suppose to, even after I called several times and asked the nurses to please take them off at night. Because of this my mom has a huge blister on top of her foot and marks around her feet and legs where the hose were cutting in to her. On top of all that, I don't think she got a bath the whole two weeks that she was there. There were several times that I tried calling her and they told me she was asleep that they would have her call me back, and I never got a call back. The only time the social worker called to update me about her was right after I asked the receptionist if she knew why I hadn't heard anything yet.

Please DO NOT take your loved one to this location..... My son was released today after 3 days.... he still had the same clothes on he came in 3 days ago, he was not even offered to brush his teeth!!!!!!!!!!!!! Can you imagine? It’s true you do NOT see where your loved one puts there head at night or any part of the facility!!! So my question is what are you hiding?? My son has been in and out of facilities for 2 years we have always been able to see the entire facility.... furthermore they wouldn’t even let me give my son a meal from the outside really?!!!!!! It took over 3 hours for the discharge. Please reasearch other facilities this is not a good choice. If I didn’t have to give 1 star I wouldn’t this place rates 0 in my eyes. The one thing that I can say about this place is the doctor was amazing and the social worker there was amazing however they need help on their staffing The very first night my son was in there they said oh we will take him a shower tonight if not tonight tomorrow morning .....that NEVER happened.......... when I picked him up on the third day he had food stuck in his mustache I thought I was going to be sick. His lips were so chapped skin was hanging off of his lips...I truly was so happy to hug and hold my son and take him home that I held my tongue. But what I wanted to say was do you have children ? Is this the way you would treat your child ? Please research other facilities DO NOT take your worst enemy to this location .

If I could give this hospital zero stars I would. Definitely substandard care. Different 'nurse' every shift. After 10 days I have yet to talk to a nurse that knows anything about my mother's day. She is hospitalized for confusion and therefore cannot tell me if she ate, slept, drank, hurts or anything. The nurses won't even look at the chart to answer my questions. I call and the phone often rings for 2 to 3 minutes and no one picks up. if they do pick up they transfer me to the nurses station and frequently no one picks up or they put me on hold for 7 to 12 min and don't come back. She smells bad and has not been bathed, her teeth are not brushed, clothes dirty. I would not recommend taking your pet to this hospital. Oh and the 2 '5 star' reviews listed on this website I'm nearly positive are from ladies who work there!!!! Unbelievable. Stay away, you have been warned.

My mother was a patient at this facility a year ago. She still has nightmares about her treatment or lack there of. There was no separation of young and geriatric. The door was unlocked. At the end of our visit one day, we pushed the call button and no one came. After thirty minutes I stepped into the hall and heard a party going on so I walked towards the noise. The entire floor staff was partying in a glass-enclosed room. Patient doors were closed. Who was on duty? They yelled at me for leaving the visiting room. It took at least two weeks to get my mom “discharged “. Only after I called the ceo was she finally moved to a different facility. This place should not be allowed to continue treating patients. Mom had good insurance and was nothing more than a cash cow to them.

My sister was at this facility about a year ago. Every bad review on here sounds just like what we went through. She has had several in patient stay over the years and nothing compares to this place! First the place seems to be on lock down and it’s like they have something to hide. When you go visit they walk you to a locked room then they bring patient to you....so you never see the room or where they are keeping the patients. My sister always seem total over medicated and total out of it. We took her clothes and she always had someone else’s clothes on which is very disturbing. If they can’t keep up with patients items how do you keep up with medications or any thing else. In the end they lost most her belongings and above all lost her new dentures. They did pay for new dentures. I feel the only reason they replaced them they knew without doubt I would be filing a complaint against them and it wasn’t stoping with the dentures!! She looked as though she had no showers for the two weeks she was there. She was always hungry and thirsty when we visit. Upon asking for something to snack on or drink for her you would think we ask them to fly us to the moon!! Seriously when a person is so thirsty and hungry every time you visit something is wrong!! When we wanted her released it took over a week . It was one excuse after another to why she couldn’t be released. The only reason she was placed at this facility was because of insurance. Never again!!!! I’m sorry to rate a one star however you can’t writ a complaint if you don’t rate them. This place is definitely a zero and should be closed! Someone needs to investigate never have we had these issues at any other facility. 000000

Very unprofessional, was told I could talk to my dad anytime I wanted but every time I called he was sleeping. I'm sure he was they drug him to keep him lethargic. I had to have police officer help me release him because he was sick and they never even called to tell me anything other than the new drugs they gave him. Was supposed to be there only a week which turned into two weeks. He is a dementia patient and has medical issue there only focus was his behaviors. Well dementia patients have behavioral problems and shouldn't be sent to this place! Sent him home with clothes that was not his. After getting home and review of discharge paperwork they have a person listed as his daughter that is not anyone in my family! My dad now is in medical hospital for pneumonia. 3 hours after forcing his release.

Unfortunately this was the only bed in the city. Horrible management, very secretive and my Father was sedated right before we went in to pick him up. We visited with him the day before and was fine. His stay was only two days but two days to long. They won't share treatment plans, confirm medication dispensed and had him wearing leftover clothes from prior patients. DO NOT GO TO THIS FACILITY PERIOD. We called and left several messages to discuss his care and the management was always busy and never called us back.

Wife was sent here from local hospital in MI, a four hour drive, due to her having multiple disabilities and was told this facility had an attached ER for any emergencies. This was not the case. She is epileptic and they were VERY ill prepared for this. She was forced to be in a bicycle helmet nearly 24/7 (they even expected her to sleep in it!) and despite being a medical place had no guard rails for beds and instead offered to put the mattress directly on the dirty hospital floor for her to sleep on. When she did have multiple seizures due to them playing with her medications they called me up to inform she was being sent to ER 20 mins away! They do not have an ER in this hospital. The list of annoyances with rude staff and mishandling of her belongings is long. They are so strict with rules they would not let her use a magnifying APP on her phone to read her large print bible even though she is LEGALLY BLIND!! The part that pushes me to a horrible review is the only time i talked with the doctor for the entire two weeks during a "family meeting" was for two mins and all that was discussed was medication changes then i talked with a social worker. They informed me they wanted to discharge her right now. Note was is 06/29/2017 around 330 PM and i am at least 4 Hours away. They do not read any files. When i laughed in their ear of how unrealistic that was, I was then told it would be the next day 06/30/17 and they would not confirm if i had to get her in person or drive there (this is the Friday before the 4th of July so MAJOR traffic). I did not learn until 8-am (the time i needed to start leaving to arrive in Indianapolis during afternoon with traffic) that they managed to find an ambulance to ride her home that will leave them around 10-pm. They seemed baffled that i was from out of state and could not just get her sooner. I am making a formal complaint with the higher ups due to the many problems with common sense i dealt with.

Oh my goodness where do I begin. This is the most corrupt facility that I have ever worked for! Once I believed that geriatric or elderly patients were most susceptible to abuse and neglect. However, since working at this facility I absolutely 100% know better. Psychiatric patients are among the most vulnerable bc of a disease process that leads people to not believe these patients and a lack understanding of mental illness. The very old and very young will always remain vulnerable, but please pay attention to your family w mental illness. The facility is almost always understaffed. Many staff have compassion and want to provide quality care, but are stressed and not able to for a lack of additional staff support. Many times patients do not have snacks bc the facility is out. Patients have gone days without showers bc there is no body soap or towels. Patients have gone with dirty clothes bc washing machines do not work. The food is catered per MCL, but I promise this slop does not in any way compare to the good food they sell in MCL cafeteria. Food is nearly cold at every meal and not supplied in sufficient amounts. Patients are frequently dehydrated bc they do not have access to water. Staff has to provide what the patients drink. You better hope that your loved one can walk and talk and pray they are lucky enough to get a staff member who cares about humanity. Many staff are not fit to care for a dog. We’ve spoken about the basic necessities like water, food and care. Now let’s talk about the medical aspect. The medications are supplied solely from an in-house pharmacy. There have been so many times that I have not administered a medication bc I don’t have it to give. If I could I’d drive to CVS and get them myself. Primarily bc they need it and also as a RN I am not able to just say that I didn’t give a med bc the med isn’t available. I can get into huge trouble for this. However, what am I to do if pharmacy will not supply a medication. Important medication like hypertension meds, thyroid and even pain medication. Let’s talk about labs. Nursing staff is solely responsible for obtaining blood to be tested. Fine no problem. In a bigger hospital this idea works. However, for Neuropsychiatric hospitals it does not bc although we as nurses do obtain the specimen it is only picked up daily and has to travel 2.5 hours to southbend medical foundation. Laboratory values are very important to determining overall health and determining the amount and types of meds that will be given. For example, a PT/INR is a blood test that is performed for patients who are on an anticoagulant for afib, blood clots or multiple other medical conditions. If too much Coumadin the blood is thin and you risk bleeding excessively. Too little and blood is thicker and can lead to embolism or blood clot which could cause heart attack or pulmonary embolism. Patients belongings are lost so many times. Expensive things like hearing aides, prosthetics ( fake arms and legs), and once a staff stole the wedding ring of a woman who was married for 40 years. I was present when it happened so I stripped down to my tee to prove that I absolutely did not take it, I wouldn’t ever do something like this, but somebody did and has to live w what they did. Money cannot replace this sentiment. Therapies do not happen. Although, I bet in the chart it will say it has happened...just ask the patient if it happened. Constant turn over! Management is very disrespectful!! In fact, the owner Cameron Gilbert has been accused and convicted of Medicare fraud! Don’t believe me look into it per South Bend Tribune pertaining to physicians hospital of Mishawaka. To outsmart the government and public they changed hospital name from Physicians hospital to Neuropsychiatric hospital. DO YOUR HOMEWORK PLEASE! Now personally, I am so scarred from working at this facility I am questioning if I ever want to practice nursing again. I’ll drive for Uber first. My son who is 20 yrs old but in his mind he is 5 or 6 due to him being autistic and epileptic, was taken to this awful place. Not by choice, this place needs shut down or all new staff. And the violent patients should not be out with other patients. My sons was only there for 3 days. There was no way we would allow our son another minute in this place. We would of got him sooner but they kept refusing his release. Finally my sons dad refused to leave without our son. My son did not get a shower not 1 time in these3 days or even get his clothes changed. They lost his shoes and clothes. A 6 ft 200 some lb guy punched my son in the face, and another guy shoved my son causing him to fall and hurt his arm and not 1 nurse done anything about it. My son has been so traumatized by this place. And this Mom will not stop until all you awful people pay for what's been done to my son!

We have had more than 1 family member here. They got nothing but the best of care. Got stabilized and they helped us find a nursing home for them n they are thriving. Staff n social workers are more than helpful. Quick to answer questions. Thank you

My hometown sent me to this facility on an EDO, even though I willingly said I would go. From what I observed and overheard when I arrived, the facility was short staffed. I felt like there were different nurses everyday, and none of them seemed to know how to handle their mentally ill patients. The negativity and hostility in the air definitely makes it a horror movie for those actually trying to seek help. They were also very neglectful. Patients in wheelchairs who needed to use the bathroom were ignored, personal items were misplaced, lost, or placed on/given to other patients. I arrived with a nice pea coat, and had to leave without it. I had to basically beg for showers and my hygiene products daily. Certain restraints were used on patients when they didn't need to be used. The food was so terrible that it made me nauseous to eat.. but I was nauseous from not eating. I drank different juices as much as possible to try and keep some nutrients in my system. I overheard CNAs making fun of patients numerous times. the facility wasn't kept as clean as it could have been. Medical doctors and therapists of some sort are supposed to talk with you daily, which they didn't. I was supposed to have bloodwork done to make sure nothing was wrong with my thyroid.. which also didn't happen. There were some positives though: some group activities were enlightening, and the main psychiatrist was excellent. Not all the staff was disappointing, but it was only about a handful. In all though - it's a very eye opening experience.. just in a different way.

The worst management I've ever experienced. The owners repeatedly tear down their staff. They several changes to staff most without just cause. They cut corners then blame you for the outcome. The state comes to visit what seems like every month due to all the complaints. Patient belongings are always disappearing. There are some staff who truly want to make a difference and provide great care but the upper management and owners will push you to quit or drive you into a stress state. The corrupt practices and the constant poor treatment was enough to quit. I wouldn't reccomend this place to anyone for work or care.

DO NOT under any circumstances allow anyone to be admitted to this horrible place. All of the negative reviews are telling the same exact story as my brother experienced. I guarantee the positive reviews are only by employees or employee family members/friends. This is a hell hole. Patients are stripped of all personal possessions. There are no phones, televisions, newspapers allowed. I was not able to speak to my brother on the phone (I live out of state) prior to my visit. Patients do not receive hygiene care and my brother was not allowed to brush his teeth for days. Although he had decent clothing with him, he was always dressed in stained, ripped items that staff said were donations. Staff said they could not get prescribed medications. This place is a private facility contracting with the state. Shame on Indiana for utilizing this facility. My brother was there two weeks and I am thankful he is still alive after being there. I wouldn’t send my worst enemy there. I know a person who is incarcerated who is treated much better. Indiana Department if Health should shut this place down immediately. It doesn’t deserve even one star but zero or negative stars is not an option.
