A couple things to point out, opinionated of course;
1: When it is known that a patient has been attending afternoon appointments regularly for 2 decades due to schedule conflict, and then instead of dropping and finding another neurologist, your patient tries to bend that schedule to accommodate you, I find it absolutely appalling that the favor can't be returned. I admit I was an hour and a half late, but I still made the attempt and drive an hour and a half to ask least respect the appointment date. 20 straight hours the day into the night before might cause a little bit of a late start the following morning. This actually brings me into my next point;
2. It is hard for an individual to call in to give head warning of delays or sudden cancellations due to emergencies when the answering service starts at 08:30, and yo allow for an appointment to be scheduled a full hour before hand.
3. If you know that patient isn't there, and you know you're not going to allow said patient to fit in anywhere else in that day, you may want to give notice by any means.
Other than these points, don't change a thing about your personality. Spot on, with what a patient needs to hear when diagnosing, not what they want to hear. Also your knowledge of your field is uncanny, and extremely valued.
